E-rate Worksheet

Should I file for E-rate?

Please complete this form to help you determine if the E-rate will benefit your library. After you answer these questions and submit this worksheet, the results of your worksheet will display on your computer. Please print the results for your records.

1.  In order to receive any federal funds, you must have an Internet policy compliant with the provisions of the Children's Internet Protection Act (CIPA).  Do you have an Internet Policy that complies with CIPA and has been approved by your local governing authority?

Yes No (If no, please stop completing this worksheet and contact the NETLS office.)

2. Do you have a Technology Plan that has been approved by the Texas State Library and Archives Commission within the last 3 years?

Yes No  (you can still file for E-rate but must have a technology plan approved by TSLAC before filing the form 486.)
